- Get a physical exam and have your doctor fill out the Medical Evaluation section OR attach a copy of a recent physical exam (within 3 years) for another school or sports activity.
If your son has not had a physical exam within three years, or you cannot find a record of one, please get a BSA Class 2 medical form and take your son for a physical exam this week. Please give me a call if you have any questions about this.
